Peripheral Cystoid Degeneration

Peripheral Cystoid Degeneration
SYMPTOMS Asymptomatic
SIGNS Hazy linear white-gray area with tiny raised yellow-white dots in the far periphery (most commonly seen in the inferior temporal and temporal quadrants)
Cystic spaces can coalesce and become a retinoschisis
WORK-UP Full eye exam with dilated retinal exam, Scleral depression, Peripheral 90D, Gonioscopy 3-mirror (using retina mirrors)
TREATMENT Peripheral cystoid degeneration without evidence of a retinoschisis: No treatment is needed and retina needs to be monitored
Peripheral cystoid degeneration with an associated retinoschisis (without evidence of retinal breaks and is not progressing towards the posterior pole): No treatment is needed and retina needs to be monitored
Peripheral cystoid degeneration with an associated retinoschisis (with evidence of retinal breaks especially outer breaks/retinal detachments and/or is progressing towards the posterior pole): Refer to a retinal specialist ASAP for further evaluation and treatment
FOLLOW-UP Peripheral cystoid degeneration without evidence of a retinoschisis: See patient back in 12 months
Peripheral cystoid degeneration with an associated retinoschisis (without evidence of retinal breaks and is not progressing towards the posterior pole): See patient back in 6-12 months
Peripheral cystoid degeneration with an associated retinoschisis (with evidence of retinal breaks especially outer breaks/retinal detachments and/or is progressing towards the posterior pole): Once patient is seen and treated by a retinal specialist and the retina is stable, the patient should be seen back in 6-12 months
ADDITIONAL LAB | TESTS None
ETIOLOGY Small microcystic spaces (composed of hyaluronic acid) found in the outer plexiform and inner nuclear layers of the peripheral retina
DIFFERENTIAL DX Snail track degeneration, White without pressure
NOTES Peripheral cystoid degeneration is the most common peripheral retinal condition
Peripheral cystoid degeneration is considered a precursor to the development of a retinoschisis
